Around five or six days after fertilisation, a blastocyst has to squeeze out of it before it can attach to the lining of the uterus. That escape is called hatching, and it is what stages 5 and 6 of the blastocyst grading scale that clinics use to describe embryo quality are recording.

Assisted hatching is the lab stepping in to make the exit easier. The HFEA, the UK fertility regulator, describes it as using acid, lasers or other tools to thin or make a hole in the zona. Older methods used an acidic solution or a fine glass needle. The American Society for Reproductive Medicine guideline says that today it is most commonly done with a laser, cutting fully through the shell, on the day of transfer.

The idea is reasonable: if a shell is thick or has hardened after freezing, a small opening might help. Whether that turns into more babies is the question trials have been asking for three decades.

On transfer day the opening is usually made with a laser, in the lab, before the embryo is loaded.

Does assisted hatching improve the chance of having a baby?

Here is what the four bodies that have reviewed it say, checked against the source documents on 13 September 2026. Decades of trials have not produced a clear benefit on the outcome that matters, which is a baby going home.

The largest dataset since points the same way. A 2025 analysis of 1,678,872 single embryo transfers in the Japanese national registry found hatching used in 55 per cent of them, with adjusted live birth rates of 21.2 per cent with it and 22.4 per cent without. The authors reported a possible benefit in one subgroup, frozen blastocyst transfers in women under 35, and worse results in others. It is a registry study, so it shows association rather than cause, and the differences are small.

Why do clinic pages say it increases success rates?

Because success rate is a flexible phrase, and the gap between two outcomes is where the marketing lives.

A clinical pregnancy is a pregnancy seen on a scan. A live birth is a baby. Cochrane found hatching may slightly raise clinical pregnancy across 39 trials, but in the 14 that followed people to birth, the edge did not hold. Reporting the first number as success is not lying. It is stopping the story halfway.

One US clinic page ranking for this search is titled around hatching increasing IVF success rates. It cites none of the four documents above.

The UK picture was studied directly. A 2020 review of UK fertility clinic websites found 24 clinics advertising assisted hatching. Nine said it improves implantation, five said it improves pregnancy rates, and not one claimed it improves live birth rates. Only one website said its effectiveness was in doubt, and none mentioned any possible negative impact. Where prices were listed, they ran from £130 to £600, with a median of £450.

That is not an accusation against any clinic. Some include it at no charge and some use it for reasons they can explain. It is a fact about incentives, worth knowing when a brochure is doing the talking.

Is hatching the same as the opening made for a PGT biopsy?

Not quite, and it is the commonest confusion in the threads we read. Genetic testing removes a few cells, and to reach them the lab opens the shell, so a biopsied embryo already has a hole.

The ASRM guideline explicitly leaves that out of scope. It says it does not consider breaching the zona for a biopsy, or collapsing an embryo before freezing, because in those cases the opening is intrinsic to the procedure. The evidence in the table is about hatching done on transfer day for its own sake. If you want to know which kind your embryos had, your clinic can tell you, and our guide to the types of assisted reproductive technology explains where genetic testing fits. What the words collapsed, re expanded and hatched mean on a thaw report, and the rates studies recorded for each, are set out in our guide to embryo hatching after thaw and what the lab report means.

What do people who have been offered assisted hatching report?

Forum accounts are testimony, not evidence, and nobody gets to run the same transfer twice to see what hatching changed. What they show is how the decision reaches patients. We read four threads in the r/IVF community, about 38 posts and comments in total, dated between January and May 2026.

In a January 2026 thread, someone returning for another frozen transfer found their new doctor at the same clinic no longer recommended hatching, calling it no longer standard. One reply had used it alongside several other changes and could not say what made the difference; two noted it is standard at one clinic and not at another.

A March 2026 thread started from the opposite end: a clinic that hatches by default and lets patients opt out. Most replies said the same of their clinic, and several tied it to genetic testing.

In an April 2026 thread, the poster remembered ticking yes to assisted hatching before retrieval, then admitted they did not really know what it meant when they checked it. The comments spent the thread untangling biopsy, grading and hatching.

The sharpest is a May 2026 thread about billing. The poster says hatching was done on embryos from two of three retrievals without paperwork they recalled signing, it was missing from the clinic estimates, and their insurer paid for the biopsy but refused the hatching charge as experimental.

Nobody in the four threads could say whether hatching changed their result, and those who credited it did so tentatively. The accounts agree with the guidelines on variation: practice differs clinic to clinic, even doctor to doctor. They part company on consent. The guidelines assume an informed choice. The threads describe a tick box, a default, or a line on a bill.

What are the reported risks of assisted hatching?

These are written straight, and they are small. What it can offer is a few neutral questions, drawn from the documents above:

Is assisted hatching planned for my embryos, and for what reason?

Is the opening part of a biopsy, or a separate step on transfer day?

When the clinic quotes a benefit, is that clinical pregnancy or live birth?

Is it charged separately, and is it on my estimate?

What does the clinic know about my situation that the guidelines do not cover?

A few written questions make the conversation with the clinic more concrete.

That last question matters most. The guidelines were written for groups. Your clinic is treating a person, and they know your history, your embryos and your protocol in a way no article can.

Frequently asked questions

Is assisted zona hatching the same as natural hatching?

No. Natural hatching happens on its own when the embryo breaks out of its outer shell before implantation. With assisted hatching, the lab makes the shell a little thinner or opens a small area to help the embryo escape. The embryo still has to finish the hatching process itself.
Add Image

Does assisted hatching hurt or change the transfer procedure?

No. You don’t feel anything different. Assisted hatching is done in the laboratory before the embryo is placed in the catheter. The actual embryo transfer is performed in the same way.

Why do some clinics do it for everyone and others for nobody?

Because the evidence is unclear enough to leave room for different house practices. ASRM says it should not be routinely recommended for all patients, ESHRE does not recommend it, and the HFEA cannot rate it for most patients. Clinics still make their own lab decisions within that.

Is there evidence it helps after a frozen embryo transfer?

It is mixed. The 2022 ASRM guideline found insufficient data to make a recommendation for frozen transfers, and a 2025 Japanese registry study reported a possible benefit in one subgroup and worse outcomes in others. Neither answers the question for an individual.

Does assisted hatching cause identical twins?

It might slightly raise the chance, but the evidence is conflicting. ASRM found roughly equal numbers of studies showing a rise and showing none, and concluded the evidence is insufficient to say definitively. The outcome is rare either way.

How much does assisted hatching cost?

It varies by clinic and country, and some clinics include it without a separate charge. A 2020 study of UK clinic websites found listed prices from £130 to £600, with a median of £450. Your own clinic’s estimate is the only figure that applies to you.

This article is general information, not medical advice, and is not a substitute for care from a qualified healthcare professional.
