This page explains where a phrase comes from and what clinics say to look out for. It does not read any twinge as a sign either way, and it cannot: your clinic knows your protocol, your scans and your medicines, and this page knows none of them.

What is lightning crotch, according to obstetric sources?

The Cleveland Clinic defines lightning crotch as “a sharp, sudden pain that happens during pregnancy”, caused when the fetus puts pressure on the cervix or the nerves around it. It says most people feel it in the third trimester, weeks 28 to 40, and that an episode does not last longer than 30 to 45 seconds. It is not a sign of labour, and the same page separates it from round ligament pain, which it places mostly in the second trimester.

So the term has a very specific job. It describes a heavy, fully grown baby sitting low in the pelvis and leaning on nerves. It was not coined for anything that happens in the first weeks of pregnancy, and the obstetric pages that rank for it never mention implantation at all, because there is nothing to connect.

Is lightning crotch a sign of implantation?

No source says it is, and the timing explains why. In the classic study of natural conception published in the New England Journal of Medicine in 1999, implantation happened 6 to 12 days after ovulation, most often on days 8 to 10. After a blastocyst transfer the embryo is already five or six days old, so the window is shorter still. At that point the embryo is smaller than the full stop at the end of this sentence.

Lightning crotch needs a baby big enough to lean on a nerve. Implantation involves a cluster of cells settling into the womb lining. Borrowing one name for the other is a little like describing a seed going into soil with the word for a tree falling over. Here are the two uses side by side, which is the comparison the ranking pages leave out:

In obstetric sources

In two week wait forums

When

Third trimester, weeks 28 to 40 (Cleveland Clinic)

In the accounts we read: hours after a transfer to about 8 days past ovulation

What is happening

The baby presses on the cervix or nearby nerves

Unknown; the phrase names a feeling, not a cause

How long

No longer than 30 to 45 seconds per episode

Anything from a single zap to several days of twinges

Read as

A normal late pregnancy symptom, not a sign of labour

Often listed as a possible early sign, alongside cramps and sore breasts

Evidence that it predicts anything

None claimed

None; no study links it to implantation

Forums are where the phrase travels, and where the quiet results rarely get posted.

Why do two week wait forums use the phrase anyway?

Because it is vivid, and because the two week wait turns every sensation into a clue. A sharp, shooting feeling low in the pelvis needs a name, and lightning crotch is the most memorable one going. It travels from pregnancy forums into trying to conceive forums, then into IVF threads, and somewhere along the way it gains a place on the lists of early signs.

Those lists have a structural problem that matters more than any single item on them. People post about twinges they later connect with a positive test. Almost nobody writes a thread to say they felt nothing at all and the result was positive, or that they felt every symptom on the list and the result was negative. Our guide to why feeling nothing proves nothing covers that silent half of the evidence. The list grows, and the absences stay invisible.

What else can cause sharp pelvic twinges after a transfer?

Several things, named in the clinical sources, and none of them can be told apart by feel alone. This is general background, not a way to sort your own pain.

Ovaries still settling after stimulation. The RCOG says it is normal to have some mild discomfort after egg collection. What the comments show is how loosely the phrase is used.

September 2025: a commenter on r/IVF described a twinge about three hours after transfer, “like lightning crotch in my uterus”, and said they now questioned every twinge as implantation or hormones.

January 2026: another r/IVF commenter used the same phrase for twinges two days after a saline sonogram, a diagnostic scan, with no embryo involved at all.

June 2026: a poster on r/TryingForABaby at 8 days past ovulation listed lightning crotch alongside weird twinges and called it symptom spotting.

July 2024: one r/TryingForABaby commenter reported lightning crotch around ovulation, before any embryo existed.

February 2025: an r/TryingForABaby commenter kept a list of everything they had felt in earlier cycles that ended in a period. Lightning crotch was on it, between itching and nausea, and they wrote the list to stop themselves reading the signs.

These are accounts, not evidence, and none of them shows what a twinge means. The pattern matches the official sources on one point: the phrase turns up before implantation, after procedures, around ovulation and in cycles that end in a period. Where the forums part from the clinical pages is in treating it as a clue at all.

The wait is shared, even when the answers have to come from the clinic.

When does sharp pelvic pain mean ringing the clinic?

This section is written straight.

The HFEA lists the signs of ovarian hyperstimulation syndrome as a swollen stomach and stomach pains, which can be severe, nausea and vomiting, shortness of breath, faintness and reduced urine output, and says to contact your clinic immediately.
