What every frozen cycle does have is four landmarks in the same order: the lining is checked, progesterone starts, the embryo is warmed and transferred, and blood is drawn. The gaps between those four are what move. Where more than one embryo went through PGT-A testing, what a euploid result actually means is usually a large part of which one gets warmed first.

The table below is the shape of a common medicated cycle. Treat it as a sketch of the order of events rather than a schedule to hold your clinic to, because the day numbers genuinely differ between units and between protocols.

Stage

Roughly when

What is happening

Cycle start

Day 1 of a bleed, or a start date the clinic sets

Scans booked, plan confirmed

Lining checks

Commonly somewhere in days 8 to 14

Ultrasound measuring endometrial thickness

Progesterone starts

Once the lining is judged ready

This is the anchor date for everything after it

Transfer

Day 6 of progesterone for a blastocyst, day 4 for a cleavage embryo

The embryo is warmed that morning

Blood test

Commonly 9 to 14 days after transfer

Measuring hCG

Second blood test

Often about 48 hours after the first

Comparing two numbers rather than reading one

Order of events in a frozen cycle. The day numbers vary by clinic and by protocol.

Why is the progesterone start date the one that matters?

Because the uterus is not counting your period. It is counting progesterone. A review of frozen transfer timing in Human Reproduction describes the implantation window as opening around six days after the progesterone rise and staying open for roughly two to four days. Everything the clinic does with dates is an attempt to land the embryo inside that window.

That is why the transfer day is counted in days of progesterone rather than days of the month. In a medicated cycle the review describes blastocysts transferred on the sixth day of progesterone support and cleavage stage embryos on the fourth. Two people can have the same transfer date and completely different cycle day numbers, and neither of them is behind.

It also explains why comparing calendars with someone else is a uniquely unrewarding hobby. You are comparing two numbers that were never measured from the same starting line.

What happens to the embryo on the morning of the transfer?

It is warmed, usually a few hours before the appointment, and then looked at. The embryologist is checking whether it survived and whether it has re-expanded, which is what a blastocyst does when it is doing well after warming. The expansion number in its grade can read differently after warming too, and our guide to what a good quality 5 day embryo blastocyst grade actually means decodes the rest of that number and those two letters. What happens next, once it is loaded into the catheter, is described step by step in our guide to what actually happens in the room during the egg transfer process.

Modern vitrification is very good at this. The American Society for Reproductive Medicine committee opinion on rapid cooling describes survival rates of embryos approaching 100 per cent with current systems, and notes that pregnancy rates are similar to or better than fresh transfer. It also stresses that results depend on the skill of the people doing it, which is a polite way of saying that laboratories are not interchangeable.

Very good is not the same as guaranteed. The Human Fertilisation and Embryology Authority puts it plainly: not all embryos survive freezing and thawing, and very occasionally none do. Clinics call you if that happens rather than letting you find out in the room.

One thing worth saying because the search results rarely do: the fertilisation part is long finished by this point. It happened in the laboratory before the embryo was ever frozen, which is why nothing you feel on transfer day is a sign of fertilisation happening.

Warming happens hours before the appointment, and the embryologist checks the result.

How long is the wait between the transfer and the blood test?

Commonly nine to fourteen days, and where your clinic sits in that range depends partly on the stage of the embryo and partly on house preference. A blastocyst is already five days further along than a cleavage stage embryo, so the same amount of measurable hCG arrives sooner after the transfer.

Clinics do not test earlier mainly because an early number is a number nobody can act on. A very low result a few days after transfer could belong to a pregnancy that goes on to be completely fine, or to one that does not, and a single reading cannot tell them apart. That is the reason your clinic draws blood twice and reads the pair, which is covered properly in our piece on what a single quantitative beta hCG can and cannot tell you.

Testing at home during this stretch is its own particular trap, because the trigger shot can still be in your system and produce a line that is true and meaningless at the same time. We wrote about the trigger shot problem separately.

The other thing to know about these days is that most of them contain nothing. Nothing is not a result. It is the most common experience of this fortnight and the least represented one online, because nobody opens a thread to report that their Tuesday was uneventful. If that is where you are, absence of symptoms is not evidence of anything, in either direction. When these days do contain something, it is usually the medication, which is why the period style cramping so many people feel around day 9 sits on the progesterone label rather than in the result.

Most days between the transfer and the blood test contain nothing at all.

Does a frozen transfer have different odds from a fresh one?

The published national figures do differ. In the regulator’s 2023 trends and figures for the United Kingdom, the average birth rate was 25 per cent per embryo transferred for fresh transfers and 33 per cent for frozen, and frozen cycles made up 45 per cent of all embryo transfers that year.

Those two percentages are not a fair fight, though, and it is worth knowing why before anyone builds a mood on them. Frozen cycles and fresh cycles are not given to the same people for the same reasons, so the gap reflects who ends up in each group as much as it reflects the freezing.

Randomised trials that do compare like with like land closer together. A trial in the New England Journal of Medicine comparing fresh and frozen transfer in ovulatory women reported live birth in 50.2 per cent and 48.7 per cent of cycles, a difference that was not statistically significant, alongside a lower risk of ovarian hyperstimulation syndrome in the frozen group.

Does a natural cycle change the dates, or just the medication?

It changes the dates, because it changes what the clinic is timing from. In a true natural cycle the anchor is your own luteinising hormone surge, with blastocyst transfer described at six days after it. In a modified natural cycle a trigger injection sets the clock instead, with transfer around seven days later. In a medicated cycle there is no ovulation to wait for, so the progesterone start date does the job. If you have been told you are on the middle one, what the trigger shot in a modified natural cycle is actually for is the part clinics rarely spell out. The choice also moves the medication line on the invoice, which is one reason a published frozen embryo transfer price rarely matches the final bill. If you are wondering why a clinic picks one route over the other, the trial evidence on medicated FET against natural cycles sets out what five randomised trials found.

Which of these you are offered usually follows from whether you ovulate predictably, rather than from preference. Researchers are still actively comparing the two approaches. The COMPETE randomised trial in ovulatory women reported live birth in 54.0 per cent of natural cycles against 43.0 per cent of hormone replacement cycles, with lower miscarriage and lower antepartum haemorrhage rates in the natural cycle group. That is one trial in one population. It belongs in a planning conversation with your own clinic before a cycle starts, not in a decision made partway through one, and your clinic knows your history, your scans and their reasons in a way that no website does.

When during this timeline should you call the clinic?

Do not wait for a scheduled appointment if something is wrong. Contact your clinic the same day for heavy bleeding, for pain that is severe or one sided, for fever, for fainting or dizziness, or for rapid abdominal swelling, breathlessness or a sharp fall in how much urine you are passing. Those last few can indicate ovarian hyperstimulation syndrome, which needs medical assessment.

For bleeding specifically, the useful questions are about volume and pain rather than colour, and we set out those thresholds in our article on clots in the two week wait. Nothing on this site can tell you what your own bleeding means. Your clinic can, and that is what they are there for.

Frequently asked questions

How many weeks does a whole frozen embryo transfer cycle take?

Commonly around three to six weeks from the start of the cycle to the blood test, though this varies with the preparation your clinic uses.
